Can I order by fax?

Following the steps below to send us your order and payment information via fax:

1.  Print this page first (you'll need these instructions and the form at the bottom).
2.  Place items in your shopping card by clicking the 'Add to Cart' button.
     (Be sure to select desired options, such as Gift Wrap, etc.)
3.  When order is complete, click the 'View Cart' button.
4.  Print the cart page(s) that display(s) your order contents.
5.  Circle the shipping method you would like to use.  It will show you the cost.
6.  Complete the form at the bottom of this page.
7.  Fax all pages (no cover sheet needed) to 678-623-0792.

Items with a bullet ( ● ) next to them are required.
